Q. Do I need a referral every time I see a specialist?

A. Yes, if your insurance is through a managed care organization, all specialist visits require authorization from your primary care physician (PCP), except for PPO plans. Certain tests and procedures may also require prior authorization. Please contact our office if you believe you need authorization for any scheduled services.

Q. How can I confirm if my referral or authorization has been completed and forwarded to the specialist or facility?

A. Once you request a referral or authorization, it will be processed within 5 business days, unless we need to contact you for further information. We will fax the referral authorization directly to the specialist or facility. In certain instances, it may be accessible online based on your insurance. You may also contact the specialist or facility to confirm receipt.

Q. Is there an expiration on my referral?

A. Yes, referrals are valid for 90 days from the date they are issued, which typically aligns with the date of your referral request. This allows sufficient time for you to schedule an appointment with the specialist.

Q. When should I request a referral?

​

A. Schedule an appointment with your PCP when you require a specialist/testing facility. Our processing time is 5 business days. Failure to notify our office in advance may result in out-of-pocket expenses for your visit or test.

In each section, the top says “Legacy Family Clinic”…we are Legacy Family Care. Can the logo not be displayed on each page since the name alone is in a smaller than expected font in the upper left hand corner? I believe this needs to be in a bigger font and preferably centered.
