Accepted Insurance

 Please ensure one of our physicians is designated as your PCP on your insurance prior to your appointment. If your physician is not listed under your insurance, please contact our office directly, and we will assist in verifying coverage.  

Please note, we do not accept Medicaid or other community plans.​ If you have Medicaid and wish to be seen at Legacy Family, we regret that we cannot accommodate you, even if you opt to pay out of pocket, as we are unable to provide referrals, send prescriptions, or order imaging due to the lack of a contract with Medicaid or community plans.  

For patients without insurance, we can offer a specialized policy for you and your family.
