Patient Financial Responsibility Policy
To ensure the continued delivery of quality medical care and to maintain fair billing practices for all patients, our practice requires that patient accounts remain in good financial standing.

Outstanding Balances
Patients will not be scheduled for appointments or seen by a provider if there is an outstanding balance on their account or on the account of an immediate family member associated with the same household. This includes balances held by a spouse or dependent children. Patients, family members, or individuals with an outstanding balance of $100 or more will not be seen, and medications will not be refilled until bill is addressed.
Partial Payments
If a patient is unable to pay the full balance at the time of service and chooses to make a partial payment, a valid credit or debit card must be kept securely on file with the practice.
Payment Timeline
Patients who make a partial payment will have 60 days to either:
-Pay the remaining balance in full, or
-Make an additional payment toward the outstanding balance.

If no payment is made within the 60-day period, the account may be subject to additional collection procedures and appointments may be restricted until the balance is resolved.

It is the patient's responsibility to understand their insurance coverage, including deductibles. Legacy Family Care is not responsible for verifying or tracking deductible amounts.
Policy Enforcement
This policy applies to all patients of the practice and is intended to ensure fairness and financial accountability across all patient accounts.
